HR, Community Health and the Wellness Committee have joined forces and formed the Employee Wellbeing Program!

Western Wisconsin Health is offering free biometric health screenings to increase employee awareness about their current health status in hopes of preventing the onset of chronic medical conditions and encourage healthier habits.  Screenings are available for all employees! Participation in this program is encouraged, but optional.

Employees with an FTE of .5 or higher are eligible to earn Wellbeing Dollars by completing and submitting the form below.

Employees with an assigned FTE of .75 or higher are eligible to receive up to $600 in Wellbeing Dollars for completing the Employee Wellbeing Program!  Employees who are enrolled in the 2020 medical plan will receive $25 off each medical plan payroll deduction.  Full-time employees not enrolled in our medical plan will receive a $50 bonus on their first paycheck of each month in 2020. 

Employees with an assigned FTE of .5-.74 are eligible to receive up to $300 in Wellbeing Dollars for completing the Employee Wellbeing Program!  Employees will receive a $25 bonus on their first paycheck of each month in 2020.

The Employee Wellbeing Program for below must be completed and submitted by November 22, 2019 to receive the Wellbeing Dollars.
